A GP mental health plan provides a structured approach for managing patients with a mental disorder. The preparation of the GP Mental Health Care Plan involves:
- Discussing assessment with patient;
- Discussing referral and treatment options with patient;
- Agreeing on goals with the patient and any identifying any patient actions;
- Provision of psycho-education;
- Crisis Intervention and/or Relapse prevention plan (if appropriate);
- Arrange for required referrals, treatments etc.;
- Document this in the patient’s GP Mental Health Care Plan

NB. It is not mandatory to use a specific form for a GP Mental Health Care Plan. GPs must ensure they have met the Medicare requirements for the item. This includes documenting the required information in the patients plan (Source: www.health.gov.au).
