HAVE YOU COMPLETED A MENTAL HEALTH TREATMENT PLAN FOR YOUR PATIENT?



A GP mental health plan provides a structured approach for managing patients with a mental disorder. The preparation of the GP Mental Health Treatment Plan involves:

  • Discussing assessment with patient;
  • Discussing referral and treatment options with patient;
  • Agreeing on goals with the patient and any identifying any patient actions;
  • Provision of psycho-education;
  • Crisis Intervention and/or Relapse prevention plan (if appropriate);
  • Arrange for required referrals, treatments etc.;
  • Document this in the patient’s GP Mental Health Treatment Plan

NB. It is not mandatory to use a specific form for a GP Mental Health Treatment Plan. GPs must ensure they have met the Medicare requirements for the item. This includes documenting the required information in the patients plan (Source: www.health.gov.au).
