There is no particular form by which a GP needs to communicate with the treating psychologist. However, the referring practitioner should provide a referral letter, and when the patient is referred by a GP who is managing the patient under a GP Mental Health Treatment Plan, a copy of the plan may also be provided (with the patient's permission) (Source: APS, 2007).
